We are a non-profit health center that provides primary care medical and dental services to all regardless of insurance status. We accept patients with or without insurance and provide health care discounts based on income and family size using the federal poverty guidelines. The vision of CHDC is to be the health care provider and employer of choice by establishing a culture that champions patient centered-care, promotes staff engagement, embraces the latest technology, and pursues partnerships and strategic alliances. Quality, Integrity, Respect, Teamwork, & Communication Community Health and Dental Care was formed through the generous funding of the Pottstown Area Health and Wellness Foundation along with various local businesses. Our doors opened in October 2008 with 39 medical and dental patients seen that first week and we've grown to see thousands of medical and dental patients each year. As a non-profit community health center, we have been organized to become the patient's medical home for preventive care and to promote overall well-being, thus reducing the number of emergency room visits.
