Flexible Home-Based Primary Care Physician

Cleveland Clinic's Center for Connected Care seeks an Associate Staff physician to join our growing home-based primary care practice serving older adults in their homes on the west side of our territory. The role blends technology-enabled care with compassionate, patient-centered service and may involve in-home visits, geriatric assessments, and supervision of NPs. This flexible opportunity supports office-based time, SNF, virtual medicine, or inpatient care as needed, with partial weekend call.
