Home Infusion Clinical Pharmacist in Carmel, IN

Responsibilities: Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment following all state and federal regulations. Assess lab values, perform pharmacokinetic dosing calculations (such as vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatment. Provide comprehensive counseling and education to patients and caregivers on specialty medications and infusion therapy. Train patients and caregivers on the use of infusion pumps, sterile techniques, and catheter care at home. Collaborate effectively with prescribing physicians, home health nurses, and internal teams including intake, reimbursement, nursing, clinical services, and pharmacy operations to ensure coordinated patient care and prevent hospital readmissions. Maintain strict compliance with regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA. Manage pharmacy inventory, including refrigerated products, and coordinate packing and shipping of medications to patients' homes.

Qualifications/Desired Experience: Minimum of two years' experience in infusion specialty pharmacy. IVIg experience is highly desirable. Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations. Current and active Indiana pharmacist license in good standing.
