Home Health Private Duty LPN/LVN

Aveanna Healthcare seeks a compassionate Private Duty Nurse LVN/LPN to provide one-on-one skilled nursing care in patients’ homes. In this role, you will assess, plan, and deliver personalized care for pediatric and adult patients, including medication administration, trach/vent support, wound care, and chronic condition management. You will collaborate with families and the care team, document accurately, and follow care plans and safety protocols. Provide one-on-one skilled nursing care in patients’ homes Perform assessments and follow individualized care plans Administer medications, treatments, and interventions Manage trach/vent, tube feeding, and respiratory support as needed Monitor patient condition and respond to changes promptly Document care accurately in accordance with clinical standards Educate and support patients and families on care routines Collaborate with physicians, therapists, and care coordinators Maintain infection control and safety protocols in the home Participate in ongoing training and adhere to Aveanna policies Skilled nursing care Patient assessment Medication administration Trach and ventilator care Wound care Electronic medical documentation Infection control Care planning and coordination Home health regulations compliance Patient and family education Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LVN or LPN license in state of practice Current BLS/CPR certification Pediatric or home health experience preferred Valid driver’s license and reliable transportation
