Job Summary:
Our agency is expanding, and we are looking for an experienced Occupational Therapist to provide home health services focused on improving patient independence and safety within their home environment. Responsibilities:
- Conduct in-home evaluations and functional assessments
- Develop and implement individualized treatment plans
- Assist patients with ADLs (bathing, dressing, grooming, toileting)
- Train patients on adaptive equipment and home safety modifications
- Complete OASIS documentation as required
- Collaborate with nurses, PTs, and other team members
Qualifications:
- Graduate of an accredited Occupational Therapy program
- Active Florida OT license
- Home health experience preferredOASIS experience strongly preferred
- Strong documentation and time management skills
- Valid driver's license and reliable transportation
