Home Health Speech Language Pathologist Become a part of our caring community as a therapist at CenterWell Home Health. Reporting to the Clinical Manager, you will play a vital role in helping patients regain strength, mobility and independence from the comfort of their homes. By delivering personalized care focused on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.
Responsibilities:
- Evaluate, direct and provide speech/language pathology service to patients in the home or facility
- Participate in the development and periodic review of the Plan of Treatment and Plan of CareUse professional skills and judgment to assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions
- Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening
- Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician
- Provide instruction and training to patients in use of alternative communication systems when appropriate
- Provide counsel and instruction to patients, families and healthcare staff
- Maintain and submit documentation as required by the company and/or facility
- Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports as required by company policy
- Participate in care coordination activities and discharge planning
- Maintain the highest standards of professional conduct concerning confidential information; share information only when the recipient's right to access is clearly established and it is in the best interests of the patient
Required Qualifications:
- Minimum six months experience as a speech therapist / speech language pathologist
- Current and unrestricted license
- Current CPR certification
- A valid driver's license, auto insurance and reliable transportation
Preferred Qualifications: Home Health experience
Compensation and Benefits
Pay Range:
$54.00 - $76.00 per visit/unit; $85,400 - $117,500 per year base pay. Benefits include paid time off, 401(k) retirement savings plan, employee assistance program, and business travel and accident coverage.
Additional Information:
This role is patient‐facing and is part of Humana's Tuberculosis screening program; TB screening is required. It is also part of Humana's driver safety program; a valid state driver's license, vehicle liability insurance and proof of reliable transportation are required.
