Recognizing a change before it becomes a crisis. THIS IS CLINICAL LEADERSHIP.As Director of Rehabilitation, you'll lead the physical therapy program while remaining clinically engaged. You'll develop your team, elevate clinical reasoning, build relationships across the community, and partner with leadership to make rehabilitation an integral part of the resident experience. You'll: Lead and develop an exceptional therapy teamMaintain an active clinical caseloadDrive clinical quality, outcomes, and evidence-informed practiceBuild strong relationships with residents, families, caregivers, nursing, and community leadershipDevelop programming around mobility, balance, fall prevention, strength, function, and independenceSupport residents across Assisted Living, Memory Care, and wellness programmingIdentify opportunities for early intervention and proactive clinical supportBuild strong referral and interdisciplinary relationshipsUse data and clinical insight to continually improve the programAnd you'll havePrava behind you.
