Schedule: 8:00 AM-4:30 PM | 36 Guaranteed Hours/Week | On-Call/Weekend: 1 Week and 1 Weekend

Contract Length: 13 Week/sPay offered is based on experience, expertise, credentialing, and education. Manage and coordinate patient care as a Case Manager. Assess patient needs and develop individualized plans of care. Coordinate services and communicate with physicians and interdisciplinary healthcare teams.
