PTA: Bring Therapy Home Community-Based Home Health

Home Health Care, Inc. is seeking a Physical Therapist Assistant to provide one-on-one therapy in patients' homes across St. Cloud and Sauk Centre areas. You will implement individualized plans of care under a supervising PT, focusing on functional mobility and safe home environments. You'll travel to patients, deliver therapeutic exercises, monitor progress, and document outcomes while collaborating with an interdisciplinary team. Minnesota PTA license required.
