Ambulatory Clinical Pharmacist - Cardiology (Hybrid)

Overview In this role you deliver comprehensive pharmaceutical care for ambulatory patients, ensuring safe, effective, and cost-conscious therapy. You collaborate with the multidisciplinary team, provide patient education, and support access to medications. You’ll participate in performance improvement, education, and precepting, with a hybrid schedule including clinic-based and remote work. You help optimize pharmacotherapy across cardiology ambulatory services and contribute to safe, patient-centered care. Responsibilities Participate in performance improvement efforts to enhance pharmaceutical care and processes Collaborate with the multidisciplinary team to monitor and adjust drug therapy for safety and cost-effectiveness Support department staffing by covering shifts as needed Ensure timely order processing through entry, verification, and resolution of issues Perform patient-specific medication profile reviews and protocol-based therapy (e.g., PK dosing, IV-to-PO conversions)Adjust dosing for organ dysfunction and monitor for adverse drug events Report medication incidents and participate in safety reviews Conduct medication reconciliation and provide patient education during transitions Share expertise via educational presentations and in-services Precept residents, students, and interns and contribute to training and orientation Maintain professional, clinical, and technical competencies Key requirements Licensed Pharmacist – State Board of Pharmacy in state of primary practice siteACLS, PALS/NRP, and/or BLS certifications as necessary Bachelor’s degree in Pharmacy from an accredited college or university Preferred: completion of an accredited PGY-1/PGY-2 residency Proven ability to work in ambulatory/clinical settings and collaborate across disciplines Interdisciplinary communication Educational and precepting abilities Adaptability and flexibility Pharmacotherapy management Pharmacy order entry and verification Medication reconciliation
