Family Medicine Physician

Rochester Regional Health seeks a board-certified or board-eligible Family Medicine Physician for an outpatient primary care practice in Clifton Springs. Provide wellness, preventive and chronic disease care as part of a collaborative team supported by care coordinators, nurses and behavioral health services. The role includes access to modern clinical resources and opportunities to precept learners. Provide outpatient primary care to individuals and families across all ages. Promote wellness and preventive care. Manage chronic diseases. Collaborate with nurses, care coordinators and behavioral health professionals. Use population health tools and the EPIC electronic medical record. Optionally precept medical students or residents.
