Primary Care Clinic Nurse (RN or LPN)

Mercy is seeking a compassionate Primary Care Nurse (RN or LPN) to deliver patient-centered care in an outpatient setting. In this role, you will perform assessments, administer medications and vaccines, coordinate chronic disease management, support providers during exams, and educate patients and families on treatment plans and preventive care. You will document accurately in the EMR, triage patient calls, and collaborate with an interdisciplinary team. Perform patient intake, assessments, and vital signs for primary care visits Administer medications, injections, and immunizations per provider orders Support providers during exams and minor procedures Coordinate chronic disease management and follow-up care Provide patient and family education on treatment plans and preventive care Triage patient calls and messages, prioritizing urgent needs Document all care accurately in the EMR and maintain patient records Follow infection control, safety, and quality standards Collaborate with physicians, advanced practitioners, and care teams to optimize outcomes Participate in continuous improvement and professional development activities Patient assessment Care planning Medication administration Vaccination and immunization Chronic disease management Electronic medical records (EMR) documentation Clinical triage (phone/in-clinic) Patient and family education Vital signs and basic diagnostics Infection control and safety protocols Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN or LPN license BLS certification ACLS preferred Experience with EMR systems (e.g., Epic) preferred
