About the role

Physical Therapists (PT) We are seeking PRN Physical Therapists (PT) and Physical Therapist Assistants (PTA) to support our skilled nursing therapy team on an as-needed basis. These roles are ideal for licensed clinicians looking for flexible scheduling while delivering high-quality rehabilitation services in a skilled nursing environment.
Key ResponsibilitiesPT Responsibilities: Perform evaluations and develop individualized physical therapy treatment plans Supervise PTA treatment when applicable Collaborate with interdisciplinary teams to support resident goals and discharge planningPTA Responsibilities:
  • Provide physical therapy treatments under the direction of a licensed PTCarry out treatment plans to improve mobility, strength, balance, and functional independence
  • Communicate resident progress and concerns to the supervising PTFor Both Roles:
  • Provide skilled therapy services to short-term rehab and long-term care residents
  • Complete accurate and timely documentation in accordance with Medicare and facility guidelines
  • Participate in care planning and interdisciplinary communication as needed
Qualifications:
  • Active PT or PTA license in AZ.Graduate of an accredited Physical Therapy or Physical Therapist Assistant programSNF or post-acute care experience preferred (new graduates welcome)
  • Strong communication, documentation, and time-management skills
  • SchedulePRN / As needed
  • Weekdays and/or weekends based on facility needs
