Work Environment: This role is primarily onsite, focusing on home care, which allows you to engage directly with patients in their familiar environment. By working one-on-one in the comfort of their homes, you'll be able to build meaningful relationships and effectively assess their unique needs and challenges. Key Responsibilities: Conduct thorough patient assessments to determine individual therapy needs and develop tailored treatment plans that focus on rehabilitation goals. Implement therapeutic interventions using various techniques, including manual therapy, therapeutic exercises, and education on adaptive equipment. Monitor and document patient progress, adjusting care plans as needed to meet rehabilitation goals. Instruct patients, their families, and caregivers on effective exercises, therapies, and the usage of any prescribed therapeutic appliances or prosthetics. Collaborate with multidisciplinary teams, including physicians and nursing staff, to ensure integrated and coordinated care for your patients. Utilize electronic medical record (EMR) systems for documentation, scheduling, and monitoring patient progress effectively. Participate in ongoing education and training initiatives to stay current with industry trends and best practices. Patient Population / Client FocusThe patients served in this role will include a wide range of clientele, from pediatric to geriatric populations. Commonly treated conditions may include post-surgical rehabilitation, orthopedic injuries, neurological impairments, and chronic pain management. Your ability to personalize each patient's care will be key to improving their functional outcomes and enhancing their overall well-being.

Qualifications: A valid license as a Physical Therapist in California. A Doctor of Physical Therapy (DPT) degree is required. Certification in Basic Cardiac Life Support (BCLS) must be obtained within 30 days of employment. Strong communication and interpersonal skills for effective collaboration with patients, families, and team members. Commitment to delivering compassionate care and fostering positive patient interactions.

Preferred Experience: Prior clinical experience in a home care setting is preferred but not required. Familiarity with EMR systems and telehealth platforms is advantageous. Any specialty certification in areas such as orthopedics or geriatrics can be an asset.

Compensation and Benefits: This role offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience.
