Job Title Contingent Clinical Care LVN/LPN PT- 30 Hrs - MCSparta, NJUtilize advanced clinical expertise to assess patient needs and deliver evidence-based care. Administer oral, injectable, intravenous, and topical medications. Perform routine assessments to monitor patient health and document responses. Evaluate complex patient conditions and monitor vital signs for abnormalities. Collaborate with interdisciplinary teams and advocate for patient needs. Manage patient access to healthcare resources and provide education on self-care and disease management. Critical Information Shift/hours: Full-time clinic hours, including every other weekend, with 6-week schedule blocks. Training hours may differ.
