RN Care Manager: High-Impact Member Care & Transitions

Horizon Blue Cross Blue Shield of New Jersey is seeking an experienced RN to join our Primary Nurse team. The role focuses on high‑risk members, planning transitions of care, and developing member‑centric plans of care to improve outcomes. You will serve as a mentor to staff, conduct care management activities across the continuum, and collaborate with physicians, hospitals, and members to ensure appropriate, cost‑effective care while supporting preventative care opportunities and screenings.
