JOB SUMMARY:
Will provide skilled Private Duty Nursing (PDN) services for individuals in accordance with theindividual's Plan of Service (IPOS). Primary areas of responsibility include delivering continuous skillednursing care (up to the Medicaid-covered maximum of 16 hours per day) to meet the individual's healthneeds as outlined in the person-centered plan and physician orders. Will be required to complete Nursing Assessments, Quarterly Nursing Reviews, Progress Notes, etc in the individual's home. Provides skilled treatments, administers medications, oversees labs and treatments as ordered. Collaborates with providers, staff, and families for a very minimal caseload to ensure integrated health care and compliance with Medicaid PDN medical criteria and documentation standards.
QUALIFICATIONS:
Education: Registered Nurse (RN) degree from an accredited college, university, or nursing program, including coursework and/or training in behavioral health, psychiatric nursing, or mental health services.
Experience:
A minimum of one (1) year of experience providing nursing services in a behavioral health/mental health/substance use disorder, intellectual and developmental disability, or related Healthcare setting is preferred. Strong verbal and written communication skills are required. Current licensure and registration as a Registered Nurse in the State of Michigan is required. Demonstrated computer proficiency and the ability to effectively utilize electronic health records and other workplacetechnology is required within six (6) months of hire. Individuals with lived experience related tomental illness, intellectual and developmental disabilities, and/or substance use disorders areencouraged to apply and their experience is highly valued.
ESSENTIAL JOB FUNCTIONS:
Demonstrates knowledge of universal precautions, standards of practice including but not limited to confidentiality and recipient rights, and emergency protocol. Demonstrates effective health care assessments and written health care plans/guidelines that adhere to "Person-Centered Planning".Demonstrates effective case recording in accordance with agency procedures. Demonstrates knowledge of policies and procedures for dispensing medications in a residential Adult Foster Care (AFC) or personal home. Provides education, crisis intervention, and individual and/or family treatment as necessary. Collaborates with other staff in diagnosing problems and evaluating the individual served. Work must comply with guidelines established by PIHP, MDHHS, Medicaid Manual, and any other regulatory/monitoring agencies that protect individuals served. Consult and work with other clinical staff and supports coordinators/case managers, outside agencies, primary care physicians, individuals served, and their support staff while maintaining effective communication and favorable relations. Will demonstrate functional computer literacy in all agency documentation. Will be required to maintain the highest degree of licensure or state registration specific to their position. Staff will be expected to participate in trainings aimed at the provision of services for individuals served with co-occurring disorders. Provides in-service training and consultation to staff, individuals served, their families, agencies, and other individuals, as required. Will work with Physician Services in completing Request for Physician Actions, nurse triage calls, obtaining vital signs, ordering labs, and assisting with tele-psychiatry (as applicable).
