About the Role:
Solace Hospice & Palliative Care Inc. is seeking a compassionate and dedicated Hospice Chaplain to join our care team in Saint Charles, IL. In this meaningful role, you will provide spiritual and emotional support to patients and their families during life's most profound moments. Responsibilities:
Provide individualized spiritual care and emotional support to hospice patients and their families across diverse faith backgrounds
Conduct bedside visits, memorial services, and end-of-life rituals in accordance with patient and family wishes
Collaborate with the interdisciplinary care team — including nurses, social workers, and physicians — to address holistic patient needs
Assess and document spiritual needs, goals, and care plans within the patient's medical record
Offer bereavement support and follow-up counseling to grieving families after the loss of a loved one
Coordinate with community clergy and faith leaders to facilitate patient-requested religious services
Participate in interdisciplinary team meetings and contribute to care planning discussions
Requirements:
Master's degree in Divinity, Theology, Pastoral Care, or a related field preferred
Board Certified Chaplain (BCC) credential or eligibility through APC, NACC, or equivalent organization preferred
Minimum of 1-2 years of clinical chaplaincy experience, preferably in hospice, palliative care, or healthcare settings
Clinical Pastoral Education (CPE) units completed (minimum 1-2 units required)
Ability to provide non-judgmental, interfaith spiritual care to patients of all beliefs and backgrounds
Strong interpersonal and communication skills with a calm, compassionate demeanor
Valid driver's license and reliable transportation for patient home visits in the Saint Charles, IL area
