Key Responsibilities: As a Licensed Clinical Social Worker at Redesign Counseling Hub, your responsibilities will include: Collaboratively developing and implementing individualized treatment plans by working closely with the healthcare team to ensure comprehensive care based on thorough psychosocial assessments. Meeting regularly with community resource groups and representing our clinic by participating in various committees focused on addressing the social and mental health needs of our patient population. Providing crisis intervention and counseling services, conducting thorough assessments for patients experiencing emotional and behavioral challenges. Maintaining ongoing communication with physicians regarding patient progress, emerging needs, and any potential issues that may affect their treatment. Ensuring accurate and timely documentation of clinical notes and progress reports in accordance with organizational policies, contributing to the continuity and quality of care. Patient Population / Client Focus In this role, you will be working with a wide-ranging patient base that spans all ages.
