In-Home Primary Care Physician

Signify Health is seeking a full-time In-Home Traveler Physician to provide comprehensive, value-based clinical assessments for patients in their homes. You will travel regionally to perform detailed histories and physicals, review medications, identify care gaps, and document risk-adjusted conditions using modern technology. Partner with care teams to coordinate follow-up, improve outcomes, and reduce avoidable utilization. Conduct comprehensive in-home medical evaluations including detailed histories and physical exams Review medications, chronic conditions, and social determinants to identify care gaps and risks Document encounters accurately in EHR with value-based and risk adjustment requirements Collaborate with care coordinators, primary care providers, and health plan teams on follow-up plans Travel regionally to patient homes while maintaining reliable, punctual visit schedules Interpret clinical data and diagnostics to inform assessment and recommendations Educate patients and families on conditions, medications, and preventive care Adhere to quality, compliance, and privacy standards in all clinical activities Comprehensive history and physical examination Chronic disease management Geriatric assessment Value-based care and risk adjustment documentation Electronic health record (EHR) documentation Care coordination and interdisciplinary collaboration Home-based care delivery Diagnostic interpretation (labs, imaging summaries) Patient and family communication Population health and quality metrics Medical Insurance Dental Insurance Vision Insurance 401k Board Certification in Internal Medicine or Family Medicine Active, unrestricted U.S. medical license (state-specific as required) DEA registration for prescribing as needed BLS/ACLS certification
