In-Home Nurse Practitioner or Physician Assistant

Signify Health is seeking a full-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for Medicare and vulnerable populations in their homes. You will review histories, assess chronic conditions, reconcile medications, and identify gaps in care while documenting findings in an EHR. Working with interdisciplinary teams, you’ll support value-based care, patient education, and care coordination. Perform comprehensive in-home health evaluations for Medicare and vulnerable populations Review medical history, medications, and social determinants of health Document findings in electronic health records with high accuracy and timeliness Collaborate with interdisciplinary teams to develop and refine care plans Educate patients and caregivers on chronic disease management and preventive care Identify gaps in care and escalate high-risk findings to appropriate providers Use data and analytics tools to inform clinical decision-making Follow value-based care protocols and quality metrics Adhere to clinical, regulatory, and privacy standards (HIPAA) Participate in ongoing training and quality improvement initiatives Comprehensive geriatric assessment Primary care or internal medicine Chronic disease management Electronic health records (EHR) documentation Care coordination Medication reconciliation Patient and caregiver education Telehealth and remote collaboration tools HIPAA and regulatory compliance Value-based care and quality metrics Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active, unrestricted Nurse Practitioner or Physician Assistant license in applicable state National board certification as NP or PA (e.g., AANP, ANCC, NCCPA) Current BLS certification Prescriptive authority as required by state regulations Valid driver’s license and reliable transportation
