Social Worker - Long-Term Care

As a Blakeford Social Worker, you will counsel residents and their families on emotional, environmental or physical observations and interventions. Conduct regular face-to-face visits with assigned residents, facilitate active participation in care planning, serve as resident liaison for social, health, and community services that meet individual needs throughout discharge planning activities, and compile performance metrics and present summary data.
