Client Care Experience Coordinator

Right at Home Hilton Head/Savannah is seeking a Care Coordinator to develop and maintain client care plans, communicating with clients, families, care partners and referral sources. You will ensure safe, meaningful care and mentor caregivers to support delivery. The ideal candidate has two years of business experience, can work independently with good judgment, and is available for on-call duties. A valid driver's license and insured vehicle are required.
