Applicants must have worked in this unit or setting within the past year to be considered for this position. Job Title Physical Therapist – Home Health LocationOwings Mills, MD | Home Health Schedule / Job Type Contract | Full-Time | Monday–Friday, 8:00 a.m. – 4:30 p.m. | Onsite / Field-BasedKey Responsibilities Conduct home visits throughout the assigned service area (primary coverage in Carroll County with support for Baltimore County), with an expected volume of approximately 6 patient visits per day Complete assessments, evaluations, reassessments, plans of care, and discharge documentation, including OASIS documentation Provide gait training, mobility assistance, transfer training, assistive device instruction, and home exercise program development Deliver orthopedic and neurologic rehabilitation services to an adult and geriatric patient population Provide patient and family education focused on safety and injury prevention Document clinical notes and maintain records using the Kinnser/WellSky EMR platform, meeting a productivity expectation of 30 points per week
