Care Coordination Registered Nurse

Mission Health is seeking a Clinical Support Coordinator Registered Nurse to coordinate patient-centered care across our Western North Carolina system. In this role, you’ll triage patient needs, support care transitions, and collaborate closely with physicians and interdisciplinary teams. You’ll leverage advanced EHR tools, contribute to quality and safety initiatives, and provide patient and family education. Coordinate clinical support services for inpatient and outpatient care teams Triage patient calls and messages, escalating clinical concerns to providers Support care transitions, discharge planning, and follow-up appointments Review clinical documentation for completeness and accuracy Collaborate with physicians, nurses, and allied health professionals to optimize care Educate patients and families on care plans, medications, and self-management Monitor quality, safety, and patient experience metrics; support improvement initiatives Use electronic health records to document, track, and communicate patient care Participate in interdisciplinary huddles and case conferences Precept and mentor new staff in clinical support workflows and protocols Registered Nursing (RN) clinical practice Care coordination and discharge planning Telephone triage and patient navigation Electronic Health Records (Epic or similar) Clinical documentation and chart review Patient and family education Quality improvement and patient safety practices Interdisciplinary team collaboration Case management principles Time management and workflow prioritization Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license in NC or compact state BLS certification ACLS preferred Case management or care coordination certification (e.g., CCM, ACM-RN) preferred
