Hospice RN Case Manager

Coordinate individualized hospice care by assessing patients, collaborating with the interdisciplinary care team, and developing care plans. Maintain accurate patient records, monitor pain and symptom management, teach patients and caregivers, and communicate regularly with the clinical manager and care team. On-call duties may be assigned. Assess patients and collaborate with the care team to develop and implement individualized care plans. Refer patients to other hospice disciplines as needed or document why referrals are not appropriate. Prepare and maintain accurate, timely patient care progress records. Record changes and outcomes related to pain and symptom management. Teach patients, families, and caregivers appropriate patient care techniques. Communicate with the clinical manager and care team, and provide on-call services as assigned.
