Home Health Physical Therapist Assistant

VitalCaring is seeking a Physical Therapy Assistant to provide home-based rehabilitation services under the direction of a Physical Therapist. You will implement treatment plans, guide patients through therapeutic exercises, monitor progress, and report clinical changes to the care team. This role includes educating patients and families on safe mobility and use of assistive devices, documenting visits accurately in the EMR, and collaborating closely with nurses, therapists, and caregivers. Provide home-based physical therapy interventions under PT supervision Implement and progress therapeutic exercise and mobility programs Observe, measure, and document patient status and response to treatment Educate patients and caregivers on safe mobility and use of equipment Communicate changes in condition to the PT and interdisciplinary team Follow care plans and clinical protocols to support optimal outcomes Maintain accurate, timely documentation in the EMR Travel to patient homes within the assigned service area Adhere to safety, infection control, and compliance standards Participate in team meetings and ongoing clinical training Therapeutic exercise implementation Gait training and transfer techniques Home health care protocols Electronic Medical Record (EMR) documentation Patient and caregiver education Vital sign monitoring Safety and infection control practices Functional mobility assessment Time and territory management Interdisciplinary care coordination Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state license or certification as a Physical Therapist Assistant Graduate of an accredited Physical Therapist Assistant program BLS/CPR certification Valid driver’s license and reliable transportation Home health experience preferred
