Community Health Advocate: Outreach, Education & Care

Adventist Health will be seeking a Community Health Worker to assist patients and families in navigating local services and resources. The role includes conducting intake interviews, providing health education, and supporting enrollment for uninsured patients under supervision. The ideal candidate will have a high school education (GED preferred) and may hold an associate/technical degree with formal CHW training preferred. Willits, CA location with full-time hours.
