Job Description:
Job Description HouseCalls Home Care is a mission-driven Licensed Home Care Services Agency (LHCSA) providing high-quality, compassionate care to elderly and disabled individuals throughout New York City. We collaborate with hospitals, physicians, and families to support patients' independence through personalized care delivered at home.
Position Overview:
- We are seeking experienced and compassionate Registered Nurses (RNs) to perform in-home visits across all NYC boroughs. As a Fee-for-Service Registered Nurse, you will play a vital role in patient care, assessments, and care coordination-helping clients live safely and comfortably in their homes.
- Compensation (Per Diem Rates)
- Initial Assessment Visit: $130 Reassessment Visit: $100 Resumption of Care Visit: $100 Supervisory Visit: $80
Key Responsibilities:
- Conduct initial and follow-up in-home nursing assessments
- Develop and update individualized care plans
- Educate patients and families on disease management and self-care
- Supervise and instruct caregivers and home health aides as needed
- Complete timely and accurate documentation in compliance with regulations
- Communicate effectively with interdisciplinary team members
Qualifications:
- Active New York State Registered Nurse (RN) license 1-2 years of experience in CHHA or long-term care preferred
- Strong clinical assessment and documentation skills
- Professional liability insurance is required
- Reliable transportation and willingness to travel between boroughs
