In-Home Preventive Care NP — Flexible, Autonomous Role

Hueman is recruiting full-time Nurse Practitioners to perform comprehensive health assessments (CHA) and health risk assessments (HRA) for Medicare/Medicaid members in homes and SNF settings across the country. The role emphasizes preventive care, early detection, and closing care gaps without ongoing primary care panel management. Expect a Monday–Friday schedule with typical hours, flexible assessment timing, and no overnight on-call requirement.
