Responsibilities:
Assign and prioritize cases while overseeing day-to-day case management operations and service delivery. Provide clinical supervision to ensure high-quality, client-centered care and compliance with agency, government, and confidentiality standards. Oversee assessments, individualized service plans, quarterly reassessments, referrals, and documentation to ensure timely and accurate completion. Supervise case management staff through weekly individual meetings, chart reviews, coaching, training, onboarding, and performance evaluations. Monitor client progress and documentation through regular chart audits, quality reviews, and program reporting. Support clients with complex needs, including mental health and substance use services, primary care, benefits, daily living supports, and conflict resolution. Coordinate with HASA and community/partner agencies to facilitate referrals, intake interviews, placements, and ongoing care coordination. Track referrals, placements, occupancy, and other program metrics; provide regular updates and reports to program leadership. Collaborate with program and maintenance staff to support safe, appropriate living environments and timely resolution of building or unit concerns. Participate in staff and community meetings and serve as manager-in-charge in the Program Director’s absence, as assigned.
Required Skills
Education and Experience:
Master’s degree in Social Work. New York State Licensed Master Social Worker (LMSW) required.
Certifications & Licenses
Required: New York State Licensed Master Social Worker (LMSW) or LMHC
Preferred: HIV/AIDS pre-test and post-test counseling certificates.
Preferred Qualifications:
Experience in HASA and/or other housing programs (congregate or scattered site).Bilingual/bicultural Spanish-speaking. Experience providing individual, family, and group counseling, particularly with HIV/AIDS-affected populations and/or individuals impacted by serious mental illness, substance use, and homelessness.
