Wound Care & Ostomy APN/PA — Weekdays, No Call

Yale New Haven Health is seeking a Nurse Practitioner or Physician Assistant to join the Wound Care & Ostomy Services team at Westerly Hospital, with a Monday–Friday schedule and no weekend calls. This full-time role supports inpatient, outpatient, and ambulatory care across two sites, using EPIC for documentation. Responsibilities include comprehensive wound care assessment and management, patient education, and coordination with the interdisciplinary team.
