Inpatient Medical Social Worker Case Manager

Kaiser Permanente is seeking an LMSW Case Manager, Social Work – Inpatient II to support patients at our Capitol Hill facility in Washington, DC. In this role, you’ll manage complex inpatient cases, conduct psychosocial assessments, develop discharge and care plans, and coordinate services across interdisciplinary teams. You will leverage modern healthcare IT systems to document care, track outcomes, and streamline communication. You’ll collaborate closely with physicians, nurses, and community providers, advocate for patient needs, and ensure safe transitions of care while complying with regulatory and organizational standards. Conduct comprehensive psychosocial assessments for inpatient populations Develop and implement individualized care and discharge plans Coordinate services with physicians, nurses, and interdisciplinary teams Facilitate safe transitions of care and linkage to community resources Document care in electronic medical record and other IT systems Monitor patient progress and adjust plans based on clinical needs Educate patients and families on treatment plans and available resources Advocate for patient rights and address barriers to care Ensure compliance with regulatory, ethical, and organizational standards Participate in case conferences and quality improvement initiatives Clinical social work Psychosocial assessment Care and discharge planning Case management Electronic Medical Records (EMR) systems Interdisciplinary team collaboration Crisis intervention Knowledge of community resources Regulatory and compliance knowledge Documentation and reporting Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LMSW (Licensed Master Social Worker) license Eligibility for DC social work licensure Clinical or medical social work background preferred Familiarity with inpatient or acute care settings Knowledge of community and social service resources
