Your Role: Home Health Physical Therapist (PT) As aPhysical Therapist in Home Health, you will provideskilled rehab servicestailored to each patient’s needs. Your focus will includefall prevention, strength building, mobility training, and functional independenceforsenior and geriatric patients .Key Responsibilities: Conduct in-home physical therapy evaluations and reassessments Develop and implement personalized rehabilitation care plans Improve strength, balance, mobility, and functional outcomes Provide therapy in home health, assisted living, and community care settings Educate patients and families on fall prevention and safe mobility techniques Document visits using WellSky (Kinnser) EMRCollaborate with interdisciplinary home health teams
