Position Details:
As a Psychiatric Nurse Practitioner, you will play a vital role in delivering high-quality mental health care to individuals living with chronic mental illness. You will work as part of a multidisciplinary team to assess, diagnose, and manage patients’ psychiatric needs in a community-based setting.
Key Responsibilities:
- Conduct comprehensive psychiatric evaluations to determine medication needs
- Document patient assessments and interactions in the electronic health record (EHR)
- Prescribe and manage pharmacologic treatments, including controlled substances
- Monitor patient progress and adjust medications as necessary
- Facilitate direct patient admissions to hospitals when appropriate
- Participate in weekly multidisciplinary team meetings to develop coordinated treatment plans
- Collaborate closely with psychiatrists, registered nurses, and other care team members
- Provide community-based services, including traveling to patient locations with team support
