Responsibilities: Respond promptly to on-call requests, triaging patient needs via phone and in-person visits. Assess patient condition and provide skilled nursing interventions for pain and symptom management. Educate patients and families on hospice care, medication administration, and comfort measures. Document all patient interactions accurately in the electronic medical record (EMR) per regulatory and agency standards. Communicate with interdisciplinary team members regarding changes inpatient status. Coordinate with pharmacies, DME providers, and other resources as needed.
