Job Details
Job Title: Home Health LPN
Location: Hammond, LA - covering Tangipahoa, Livingston and St. Helena parishes (counties)
Setting: Home Health
Pay Range: 1500-1700 weekly
Schedule: 08:00 - 17:00
Contract Length: 6 weeks
Candidate Type: Traveler or local candidates accepted
Requirements: 1 year of Medicare certified Home Health experience and HCHB EMR experience
Responsibilities:
Provide home health nursing care to patients in their homes.
Monitor and record patient conditions and report findings to healthcare team.
Administer medications and treatments as prescribed.
Educate patients and families on health management and care procedures.
Document patient care activities accurately and timely.
