Schedule: Monday-Friday, 8:00 AM-5:00 PM, with one weekend every sixth week
Workload: 30 units
Position Overview:
- Seeking a Physical Therapist to provide home health services across the Silverdale, WA service area.30 unitsHH requiredHCHB Required
- North and South areas north is Bainbridge isle, Kingston, Hansville Poulsbo areas and south is Bremerton, Port Orchard and Olalla areas32 hours guaranteedHCHB and Oasis required
- Required Qualifications & Skills
- Current Washington State Physical Therapist license - Required
- Current CPR certification - Required
- Valid driver's license - Required
- Reliable transportation and current auto insurance - Required 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home health/home care experience - PreferredHCHB (HomeCare HomeBase) EMR experience - Preferred
- EducationBachelor of Science in Physical Therapy from an American Physical Therapy Association (APTA)-approved program - Required
Key Responsibilities:
- Provide physical therapy services to patients in their homes.
- Evaluate patients and develop individualized treatment plans.
- Deliver therapeutic interventions based on patient needs and physician orders.
- Monitor patient progress and adjust treatment plans as appropriate.
- Document assessments, treatments, progress, and outcomes accurately.
- Collaborate with physicians, nurses, caregivers, and other members of the healthcare team.
- Educate patients and caregivers on appropriate exercises, mobility, safety, and home programs.
- Travel throughout assigned North or South service areas to provide patient care.
