In-Home Geriatric Care Coordinator for PACE

You will assess needs, develop care plans, and coordinate home care services in collaboration with the IDT to provide safe, person-centered care in the home setting. Role requires travel and a valid driver’s license; 80% of responsibilities occur in the community with required documentation in EHRs and adherence to policy.
