NP Provider Hospice & Palliative Care

MJHS is seeking a compassionate Nurse Practitioner to provide hospice and palliative care across the greater New York City area. In this role, you will perform comprehensive assessments, manage complex symptoms, prescribe and adjust medications, and develop individualized care plans in collaboration with physicians, nurses, social workers, and chaplains. You will educate patients and families on disease trajectory, goals of care, and advance care planning, while ensuring comfort, dignity, and cultural sensitivity. Develop and adjust individualized care plans focused on comfort and quality of life. Manage pain and complex symptoms, including prescribing and titrating medications. Collaborate with an interdisciplinary team to coordinate holistic patient and family care. Educate patients and families on disease trajectory, prognosis, and care options. Facilitate goals-of-care and advance care planning discussions. Document all assessments, plans, and interventions in the EHR accurately and timely. Participate in on-call rotation and home, facility, or inpatient visits as needed. Ensure compliance with hospice and palliative regulatory standards and MJHS policies. Contribute to quality improvement, education, and professional development activities.
