Home Health Medical Social WorkerOur client is seeking a compassionate Home Health Medical Social Worker with a Master's in Social Work and relevant experience to join their care team. The role involves collaborating to provide patient care plans and supporting patients in any of these locations: Kansas City, MO, Lansing, MI, Jackson, MI area.
Key responsibilities include:
Collaborating to provide management and delivery of patient care plans
Supporting patients in any of these locations: Kansas City, MO, Lansing, MI, Jackson, MI area
We are looking for compassionate Home Health Medical Social Workers with:
- Master's in Social Work from graduate school accredited by the Council of Social WorkOne (1) to two (2) years post graduate social work experience in a health care setting
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
- Current driver's license and ability to spend ~20% of your day driving to/from patient locations
- A commitment to consistently meet critical deadlines for charting
- The skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
