Hospice Admissions RN & Care Liaison

Southern Care New Beacon Hospice is seeking an experienced Hospice Clinical Liaison to manage referrals, admissions, and coordination with hospitals, physicians, and discharge planners. The role focuses on educating families about hospice, ensuring compliant intake, and building strong referral partnerships. The ideal candidate has RN licensure with 3–5 years in hospice, strong communication, and the ability to supervise and collaborate with interdisciplinary teams to ensure timely patient care.
