Part-Time Wellness Visit Provider for Medicare Patients

Axis Community Health is seeking an Annual Wellness Visit provider to conduct annual wellness visits for Medicare patients in a part-time capacity at our Pleasanton clinic. The role involves screenings, risk assessments, patient education, and collaboration with a multidisciplinary team. Applicants must have an MD/DO/PA/NP degree, California license, DEA, and be able to work with EHR systems (EPIC). Bilingual English/Spanish is highly desirable, with 16 hours per week schedule.
