Work Environment: Operating within a collaborative outpatient office, the clinic fosters a supportive environment focused on teamwork and professional development. You will be working Monday to Friday from 8 AM to 3 PM, engaging in a vibrant setting where continual learning is encouraged and the well-being of our clients is prioritized. Our approach promotes the sharing of ideas and techniques among staff to enhance the therapeutic experience for both clients and practitioners.

Key Responsibilities: As an outpatient Physical Therapist at Vitality Care Clinic, your responsibilities will include: Conducting comprehensive physical therapy assessments for patients, including children, to identify their specific needs and develop tailored therapy plans. Designing, implementing, and adjusting individualized treatment plans based on ongoing assessments and patient progress towards established goals. Regularly evaluating the effectiveness of therapy interventions by tracking clinical signs and symptoms. Maintaining accurate and timely documentation of patient progress through SOAP notes, treatment summaries, and care plans using our electronic record system. Providing education and support to patients and their families regarding the rehabilitation process, including setting realistic expectations and addressing any concerns. Collaborating with interdisciplinary teams, including other therapists and healthcare professionals, to provide holistic care for clients receiving multiple therapy services. Contributing to achieving clinic benchmarks and goals for service utilization while promoting the core values and culture of Vitality Care Clinic. Patient Population / Client FocusOur primary clientele includes a diverse range of patients from various backgrounds, age groups, and diagnoses. You will have the opportunity to work with children needing rehabilitation for developmental issues, as well as adults recovering from surgeries or chronic conditions. Qualifications: To be considered for this position, candidates must possess: A Bachelor's Degree in Physical Therapy from an accredited program, or a Doctor of Physical Therapy (DPT) degree. An active, unrestricted license to practice physical therapy in the state of California. A willingness to conduct evaluations and provide treatments both in-person and through telehealth as needed. A strong commitment to working with a varied patient demographic, including individuals from different ages, races, and ethnic backgrounds. Excellent adaptability, showcasing flexibility in a rapidly evolving healthcare environment. A collaborative mindset, with an emphasis on providing exceptional customer service.

Preferred Experience: While the minimum qualifications are outlined, experience in working with diverse patient populations and familiarity with electronic health record systems will be considered assets.
