Start Date:
ASAPResponsibilitiesAssess functional needs and implement individualized therapy plans. Provide therapeutic interventions and monitor patient progress. Document outcomes and coordinate treatment goals with the care team.
Requirements:
Active professional license or certification required for the role and assignment state, where applicable.
Relevant experience in Physical Therapist.
Current specialty credentials and BLS if required by the facility.
Ability to work the listed schedule and meet facility onboarding requirements.
