Impactful Bilingual Rapid Rehousing Case Manager

Colonial Group seeks a bilingual Case Manager to provide coordinated stabilization and time-limited case management to English- and Spanish-speaking clients in housing after exit from unsheltered homelessness or transitional shelter sites. You'll work with a close-knit team including case managers, navigators, nurses, and housing staff to ensure rapid exits into housing and coordinated follow-up for the first year.
