Home Health Registered Nurse Case Manager

CarePartners, a Connecticut-based home healthcare provider, is seeking an RN Primary Case Manager to coordinate and deliver skilled nursing care in patients’ homes. In this role, you will manage a caseload of adult patients, complete comprehensive assessments, develop and update individualized care plans, provide direct clinical care, and educate patients and families on disease management and medications. You will collaborate closely with therapists, aides, and physicians, ensuring timely communication and documentation. Manage a caseload of home health patients and serve as primary clinical contact Perform comprehensive in-home nursing assessments and ongoing reassessments Develop, implement, and update individualized care plans based on patient needs and goals Provide direct skilled nursing care, including wound care, medication management, and IV therapy as appropriate Educate patients and families on disease management, medications, and home safety Coordinate services and communicate regularly with physicians, therapists, and home health aides Document all care, assessments, and communications accurately in the EMR within required timeframes Monitor patient progress, identify changes in condition, and adjust care plans or escalate concerns promptly Ensure compliance with regulatory, payer, and agency standards of care Participate in team meetings, case conferences, and ongoing professional development Home health nursing Patient assessment Care plan development Chronic disease management Wound care IV therapy and medication administration Electronic medical records (EMR) Care coordination Patient and family education Regulatory and documentation compliance Interdisciplinary collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in CT BLS certification OASIS experience preferred Home health or case management certification a plus
