RN Case Manager Full-Time & PRN for Newnan Thomaston McDonough Area

Essential Functions: Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; and assumes the responsibility for coordination of care.

Education and Experience: Requirements Graduate of an accredited Diploma, Associate, Baccalaureate, or Masters School of Nursing. Two years of experience as an RN in a clinical care setting preferred. Knowledge and experience with end-of-life disease processes and terminal symptom management. Computer literate, proficient in Word. Must have strong communication, critical thinking, and problem-solving skills.
