Home Health Registered Nurse

AccentCare seeks a compassionate Registered Nurse to provide weekend home health visits to adult and geriatric patients. In this role, you will complete in‑home assessments, develop and update care plans, administer medications and treatments, and provide wound care and IV therapy as needed. You will coordinate with physicians, pharmacists, and interdisciplinary team members to ensure safe, high‑quality care. RNs educate patients and families on disease management, medications, and home safety, while accurately documenting in an EHR system and adhering to strict quality and infection‑control standards. Provide weekend home health nursing visits to adult and geriatric patients Complete comprehensive in‑home patient assessments and update care plans Administer medications, IV therapies, and wound care per provider orders Educate patients and families on disease management, medications, and home safety Document all care accurately and promptly in the EHR system Coordinate with physicians, pharmacists, and interdisciplinary team members Monitor patient response to treatment and escalate changes in condition Adhere to infection‑control, safety, and quality standards during all visits Patient assessment Care planning IV therapy and medication administration Wound care Electronic health records (EHR) documentation Care coordination Patient and family education Infection control Time management Home safety evaluation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in state of practice BLS certification ACLS preferred Home health or community nursing experience preferred
