Minimum Requirements:

Allied Health License for AZ or Compact as applicable
Applicable recent experience in this work setting
Commitment to quality care and adherence to client legal and client protocols

Certifications: Doctor of Pharmacy or a Bachelor of Pharmacy degree; current valid and unrestricted license to practice Pharmacy; BLS

Experience: 2 years
